What to Bring to Your Consultation

To help your first visit go smoothly, please bring the following information with you:

  • Your referral slip and X-rays, if applicable
  • A list of any medications you are currently taking
  • Any known allergies or medical conditions
  • Medical or dental insurance information to be filed

What Happens During Your First Appointment

During your consultation, we’ll review your medical and dental history and perform a thorough evaluation of your teeth and gums. Depending on your needs, this may include digital X-rays and a comprehensive dental exam. Dr. Curtis will then discuss his findings and recommend a personalized treatment plan.

Occasionally, treatment can be completed the same day as the consultation. However, complex treatment plans or complicated medical histories may require additional evaluation and a second appointment for treatment.

Let Us Know About Medical Conditions

Please tell our office ahead of time if you have a medical condition that may be a concern before treatment, such as diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, or blood disorders. This allows us to plan your care safely.

Sending Recent X-Rays

If you’ve had X-rays taken in the last six months, please have your previous dentist forward them to our office. If there isn’t enough time to send them before your appointment, you’re welcome to pick them up and bring them with you.

Patients Under 18

A parent or guardian must accompany all patients under the age of 18.

If you have questions about your first visit, our New Patients page and Financial Information page are great resources, or you can contact us today at (918) 272-5387.
